http://www.google.com/search?q=%22death+book%22+uru has references to Shomat (DRC journal?), such as this one:
In 387 DE both his brothers disappeared and were never found again. It was commonly presumed that Shomat wrote a Death Book to which both of his brothers were linked into although it could never be proven.

[edit] "It is possible"

BoT describes very clearly that the idea of these books linking to hostile places is true (showing the final moments of A'Gaeris), not just possible... Arancaytar 02:13, 24 April 2008 (CDT)

I think the reason it is called "possible" is because there might be another way to make a death book. Aitrus used the linking-to-hostile-place technique, but that might not have been the standard way to do it. Gorobay 07:02, 24 April 2008 (CDT)
